| Section II – Certification |
| Companies accepted for Regular Membership in NADCA must have a minimum of one full-time staff member
who has been certified by NADCA as an Air Systems Cleaning Specialist (ASCS).

| Section III – Liability Insurance |
| Companies accepted for Regular
Membership must maintain a minimum of $500,000 in liability insurance at all
times. To verify that your company maintains such insurance, NADCA requires
that you include a current, valid Certificate of Insurance issued by your
insurance company naming NADCA as a Certificate Holder. |

| Section IV – State Licensing(US-based Applicants Only) |
| Some states in the USA require an HVAC
contractor’s license, mechanical contractor’s license, sheet metal license, or
other specialty license in order for a firm to access and clean HVAC systems.
Arkansas, California, Florida, Georgia, Michigan, and Texas are among these
states, though other states may have similar requirements. If your state
requires a license for accessing and cleaning HVAC systems, please include a
copy of the license or a letter from the state with this application. |

| Section V – Membership Dues Payment
& Application Fee |
| Your company’s payment of one full year’s membership dues
is required along with this application, regardless of whether or not your company has an ASCS on staff.
Dues are invoiced each year in May, payable by July 1. If your application is received in any month other
than the month of July, you will receive a pro-rated invoice the following May. NADCA Membership dues are
non-refundable.
In addition to one year’s membership dues, a one-time $100 application processing fee is required
with this application. This is a non-refundable fee. Your payment of membership dues, application
processing fees and multiple listing fees (if applicable), may be made by check, money order, or credit
card. If using a credit card, please provide the credit card information below. Full payment must be
remitted with this application.
NADCA offers a corporate membership. Some corporations operate out of two
or more physical locations. In order to ensure proper listings within NADCA
directories used for consumer referrals, as well as to provide information
throughout your company, it is a
mandatory
NADCA membership requirement that all members list all of their physical
locations through which air duct cleaning services are offered. Associated with
each physical location after the first office, a $250 per office charge is
payable annually. Each office shall be listed in the NADCA Membership
Directory & Buyer’s Guide, will receive all NADCA mailings, and will be eligible for all
membership benefits. Each location must have a certified ASCS on staff, carry
appropriate insurance, and meet any state license requirements.

| Section VI - Submittal |
| I, the undersigned, certify that all the
information provided in this application is true and accurate. The National Air
Duct Cleaners Association, its officers, directors, committee members,
employees, agents and divisions (“NADCA”) may review my application and I will
cooperate promptly and fully in such review. I will submit to NADCA such
documents and information deemed necessary to confirm the information in this
application. All documents submitted to NADCA are the property of NADCA and
will not be returned to me. NADCA may communicate any and all information
relating to my membership application, membership status and any pendency or
outcome or disciplinary proceedings to state and federal authorities, and
others. I waive all claims relating to or rising out of review by NADCA of this
application, and I indemnify, release, discharge and exonerate NADCA for any
action taken relating to such review, including denial of my application,
revocation, suspension or other sanction for any action taken pursuant to the
rules and standards of NADCA. I understand that NADCA will deposit the dues
payment remitted with this application immediately upon its submittal, but that
deposit of the payment does not signify acceptance into the membership.
I understand that I shall be notified of membership acceptance by NADCA in writing
and that receipt of this application by NADCA and the deposit
of dues DO NOT constitute approval of membership.
We further understand that NADCA membership dues are non-refundable. We also understand
that certification examination fees are non-refundable, regardless of the outcome of the
examination (i.e., whether the candidate passes or fails). We agree to meet all membership
criteria within 12 months of applying and we understand that failure to meet such requirements
within 12 months will result in rejection of our application and forfeiture of all fees and dues.
I shall not make any claims of holding NADCA membership until such official notification, including an official
Membership Certificate, is received from NADCA. I also agree not to use the NADCA logo until my application is
approved, and I understand that unauthorized use of the logo may result in rejection of my application and
forfeiture of all payments, as well as further legal action. By signing, I acknowledge that I have read and
understand the information in this application and agree to abide by these terms and rules.
NADCA is recognized as a non-profit 501(c)(6) organization by the U.S. Internal Revenue Service. As such, membership dues, gifts, or contributions are not deductible as charitable contributions for federal income tax purposes. |

NADCA Code of Ethics
- We will serve our customers with integrity and competence.
- We will perform our work using Source Removal methods. In order to eliminate or prevent the delivery of airborne pollutants into indoor air spaces, through HVAC ductwork, we will remove the dust and others contaminants from the system. Thus, we pledge to administer Source Removal methods, cleaning the system ductwork as thoroughly as possible, before applying any coatings or treatments, including deodorizers, disinfectants, sealants.
- We will be honest and forthright in our advertising.
- We will provide our clients with accurate inspections and evaluations of the cleanliness and physical condition of their HVAC systems, using this information to determine the type of cleaning and maintenance services required, if any.
- We will provide only necessary and desired services to our clients, and will not use furnace/air duct cleaning as a means of selling unnecessary or unwanted products or services.
- We will provide services only after completing the necessary bonding and licensing procedures.
- We will utilize the services and products of those who possess specialized skills, tools, or trades not possessed by us when circumstances call for work to be done which we are unable to perform.
- We will stay abreast of new developments in technology, tools of the trade, building codes, the Uniform Mechanical Code, and any other codes or information that directly affects our work.
- We will require that all employees of our firm practice furnace/air duct cleaning in accordance with NADCA Guidelines and the Code of Ethics.
- We will perform our services in accordance with the current published standards of the association.
The Ethics Committee shall hear and investigate charges of unethical conduct between concerned parties, and will make recommendations to the Board of Directors concerning the resolution of those charges.
